| Eligibility |
| • | You are eligible for the Lifetime LASIK Program if you are treated at an Indiana LASIK Center for laser vision correction of myopia, hyperopia, and/or astigmatism. Eligible retreatments will be performed at a reduced procedure fee.

| • | Prior to your procedure with Indiana LASIK Center your myopic correction is between -.50 D and -12.00 D or +0.50 to + 5.00 with no greater than 4.00 D of astigmatism, and your best corrected acuity is 20/40 or better.
|
| • | You have completed the course of post-treatment care, including all follow-up visits, as prescribed by your doctor.
|
| • | Following completion of all your prescribed post-treatment care, you have annual comprehensive eye exams with an Indiana LASIK Center affiliated doctor. You (or your insurance) are responsible for the doctor's regular annual fees.
|
| • | Any refractive condition of .75 Diopters greater than your targeted outcome that exists or develops after the procedure may be retreated at a reduced cost of the original procedure fee provided your doctor agrees that an enhancement is medically safe, appropriate, and has a high probability of correcting your vision to at least 20/40 (enabling you to pass the eye test for your driver's liscense exam without corrective lenses). Any disputed cases will be reviewed by the Indiana LASIK Center Medical Review Board. top

| Enrollment is automatic when your initial laser vision correction for myopia and/or astigmatism is completed at an Indiana LASIK Center. It's that easy! Your doctor will maintain the appropriate records to determine your eligibility. top |
Exclusions |
| There are certain exceptions and conditions when additional laser vision correction will not be advised and will not be covered by the Lifetime LASIK Program. |
| • | Patients who fail to comply with the guidlines and recommended course of treatment by their doctor, including annual comprehensive eye exams as previously defined.
|
| • | Patients whose affected vision is a result of trauma to the eye.
|
| • | Patients with diseases such as diabetes, active auto-immune, and collagen vascular disorders.
|
| • | Patients whose cornea is too thin for successful retreatment.
|
| • | Patients who had RK, ALK, PKP (corneal transplant), or any other previous Laser vision correction procedure performed at a site other than an Indiana LASIK Center.
|
| • | Patients diagnosed with any ocular disease (cataracts, glaucoma, diabetic retinopathy, etc.) which would compromise a successful retreatment.
|
| • | Patients seeking an enhancement as a result of presbyopia (a normal aging process in which the muscles controlling the lens lose their elasticity).
|
| • | Patients who develop drug induced myopia.
|
| • | Patients who develop myopia as a result of pregnancy or nursing; (retreatment will be considered after the pregnancy or nursing period when vision restabilizes).
|
| • | Patients with pre-treatment degrees of myopia greater than -12.00 D; patients whose pre-treatment astigmatism was greater than 4.00 D; patients with pre-treatment degrees of hyperopia greater than +5.00 D. top |
